Target Overview
HAI-2 (Hepatocyte growth factor activator inhibitor type 2), encoded by SPINT2, is a 252-residue type I transmembrane Kunitz-type serine protease inhibitor also known as placental bikunin (UniProt O43291). The mature protein contains two tandem Kunitz domains (KD1 and KD2) tethered to the cell membrane via a single-pass transmembrane segment, with the inhibitory domains projecting into the extracellular space. HAI-2 is broadly expressed in epithelial tissues, including intestine, kidney, pancreas, and placenta, and frequently co-localizes with its membrane-anchored serine protease targets at the apical surface of polarized epithelia. Functionally, HAI-2 inhibits HGFAC (hepatocyte growth factor activator), plasmin, plasma and tissue kallikrein, and the type II transmembrane serine proteases TMPRSS13, ST14/matriptase, and PRSS8/prostasin. Through restraint of matriptase and prostasin, HAI-2 governs pericellular proteolysis required for HGF/c-Met activation, epithelial barrier integrity, and sodium channel regulation. Loss-of-function SPINT2 mutations cause congenital sodium diarrhea, and altered SPINT2 expression has been documented in multiple carcinomas, where it can function as a tumor suppressor by limiting matriptase-driven signaling. More recently, HAI-2 has drawn attention as a regulator of TMPRSS2, the host protease responsible for SARS-CoV-2 spike priming, making it a target of interest in both oncology and viral entry research.
